How do I apply for this program?

To qualify for the benefits of Project Endeavor, please download the application form, read it, sign it and either fax the signed copy to (605)782-8446 or scan the signed copy and email to info@projectendeavor.org. If you are sending your application electronically, please make sure it is in PDF (preferred) or JPG format. The contact center will not be able to process the application unless it is in one of these formats.
